On Thursday, September 19, 2019, ODFI 0410-3604-6 initiated duplicate ACH debits for company name CMS Medicare with a settlement date of September 20th. We expect RDFIs to return duplicate transactions received and appreciate your efforts to do so promptly. The ODFI will initiate reversals for any duplicate transaction not returned by end of day Wednesday, September 25th.
We encourage financial institutions to recognize this was an origination error and to work with their impacted customers, including waiver of any NSF or other penalties that might otherwise apply.
